If a nomal workplace doesn’t put up simple signs it gets in lots of trouble, why Japanese nuclear plant so lax?

In Australia you can get in trouble for not putting up a sign saying not to smoke near a doorway, even if it’s a simple tobacconist shop. In Japan a Nuclear Reactor had only 1 stretcher and an out of date radiation kit. Was this already a disaster before it happened?
